Acute undifferentiated febrile illnesses (AFIs) are responsible for substantial morbidity and mortality globally. Today, the most effective treatments for patients affected by AFIs rely on the identification of the causative infectious agents, examples of which include Chikungunya (CHIKV) and Dengue (DENV).
The initial diagnosis is typically performed using a rapid diagnostic test (RDT), based on lateral flow principles, which yields quick results while at the same time, assuring relatively high sensitivity and specificity of the infectious agents. However, while being very effective at the insurgence of the disease, the test results are ineffective at later stages, when the concentration of the infectious agent falls below the detectable range.
At later stages of the infection, diagnosis is performed using a serological test that relies on a single RDT strip for the detection of the Immunoglobulin M (IgM) antibodies to the infectious agent, which is supplemented by the Immunoglobulin G (IgG) antibody detection to indicate a past infection. As the RDT strip uses only two test lines (one for IgG and one for IgM), detection is limited to that associated with a single infectious agent. Therefore, detection of IgG/IgM associated with other pathogens would require additional pathogen-specific strips, making the entire diagnosis long, laborious and patient-specific.
In view of the above limitations, the authors developed a novel lateral flow assay scheme for multiplexing the detection of several pathogen-specific IgGs/IgMs on a single strip using two color labels based on latex nanoparticles: blue for IgG(s) and red for IgM(s) detection, while the specific pathogen is detected by the line location along the strip.
